Viagra for performance anxiety can help with the erection, not the nerves behind it. In the trials reported in the UK product licence, 84% of men with psychogenic erectile dysfunction reported improved erections on sildenafil (Viagra SmPC, 2025). It is a prescription-only medicine, and on its own it does nothing for the anxiety.

1. Sildenafil treats the erection, not the anxiety

Sildenafil improves blood flow into the penis once you are aroused, and it has no calming effect on the mind.

The product licence is blunt about this. In order for sildenafil to be effective, sexual stimulation is required (Viagra SmPC, revised December 2025). The tablet blocks an enzyme called PDE5, which lets the blood vessels in the penis stay relaxed for longer once arousal has started. It does not start arousal and it does not lower adrenaline. If you are so wound up that arousal never gets going, the tablet has nothing to work with.

A 2020 review in Sexual Medicine Reviews put it plainly. No treatment for sexual performance anxiety itself is definitively proven, but PDE5 inhibitors such as sildenafil are effective for the psychogenic erectile dysfunction it causes (Pyke, 2020). The medicine treats the symptom the anxiety produces. The anxiety is a separate job, and item 6 covers it.

The licence also reports erections in response to stimulation 4 to 5 hours after a dose (Viagra SmPC, 2025), so there is no need to rush. The window is wide. What it cannot supply is the stimulation itself.

Why it matters: You know what you are actually buying, and you stop expecting a tablet to do a therapist’s job.

2. Performance anxiety is a recognised cause of ED, and a common one

Between 9% and 25% of men experience sexual performance anxiety, and it is a documented driver of psychogenic erectile dysfunction (Pyke, 2020).

The figures are from Pyke (Sexual Medicine Reviews, 2020). The British Society for Sexual Medicine guideline describes the mechanism as an interplay among the cognitive, affective, behavioural and physiological responses throughout a sexual situation (Hackett et al., 2018). In plain terms, you worry, your body notices, the erection fades, and next time you worry harder.

It skews young. A clinic study of 439 men found that 26% of new ED patients were 40 or under, and that group had far fewer medical conditions, 9.6% against 41.7% (Capogrosso et al., Journal of Sexual Medicine, 2013).

Clinicians ask about one physical clue. The BSSM guideline notes that nocturnal and early awakening erections are a normal physiological event in all men, and overnight measurement is used to confirm normal erectile potency (Hackett et al., 2018). If yours are fine, the mechanism probably is too. The NHS says occasional erection problems are usually caused by stress, tiredness or too much alcohol, and persistent problems belong with a GP (NHS, reviewed July 2023).

Why it matters: If you are under 40, healthy, and still waking up with erections, anxiety is a likely suspect, and it is treatable.

3. Anxiety-driven ED responded better than any other type in the licensing trials

In the studies behind the UK licence, 84% of men with psychogenic erectile dysfunction reported improved erections on sildenafil, a higher rate than for organic causes (Viagra SmPC, 2025).

The Viagra SmPC (2025) reports improvement by cause: psychogenic 84%, mixed 77%, organic 68%. Across all causes, 62% of men on 25 mg, 74% on 50 mg and 82% on 100 mg reported improvement, against 25% on placebo. One in four men improved on a tablet with nothing in it, which tells you how much expectation is doing in this condition.

This is the honest case for viagra for performance anxiety. Fear of failure feeds the failure. A run of reliable erections takes the fear out of the room, and that is the logic behind using it for a short period, agreed with a prescriber, while the pattern breaks. The BSSM guideline notes that the potential advantages of lifestyle changes may be particularly pronounced in psychogenic ED (Hackett et al., 2018). No outcome is guaranteed, the dose is a prescriber’s decision, and the European Association of Urology guideline is careful to say most cases are of mixed aetiology (EAU, 2025 edition). But the data favours this group.

Why it matters: The evidence says the odds are in your favour, as long as you use it properly and the anxiety gets its own attention.

4. A rushed first attempt is the main reason it seems to fail

The BSSM guideline names inadequate prescribing or instruction as the major cause of PDE5 inhibitor failure, and asks for at least four properly timed attempts before writing the medicine off.

The guideline is specific: a minimum of 4, preferably 8, attempts at the highest tolerated dose, with adequate sexual stimulation, before a man is classed as a non-responder (Hackett et al., 2018). One nervous, hurried attempt tells you little.

Timing is where most of the mistakes happen. The licence says take it about one hour before sexual activity (Viagra SmPC, 2025). In a fasted study, the median time to an erection firm enough for intercourse was 25 minutes, with a range of 12 to 37 minutes, and the drug still produced an erection in response to stimulation 4 to 5 hours after the dose. A high-fat meal delayed peak blood levels by about an hour and cut them by 29%. The NHS adds that a large meal slows things down and that drinking too much alcohol can make getting an erection more difficult (NHS, reviewed July 2026).

Anxiety adds one more variable. If you spend the hour checking for signs, arousal never starts, and item 1 applies.

Why it matters: You give the medicine a fair test instead of deciding after one bad night that nothing works for you.

5. Some men must not take it, and the list is not short

Sildenafil is contraindicated with nitrates and riociguat, and in several heart, liver, blood pressure and eye conditions.

The full contraindication list in the Viagra SmPC (2025) is:

  • hypersensitivity to sildenafil or to any of the excipients
  • any nitrate or nitric oxide donor, including amyl nitrite (sold as poppers), as sildenafil potentiates their blood pressure lowering effect
  • riociguat
  • men for whom sexual activity is inadvisable, such as those with severe cardiovascular disorders including unstable angina or severe cardiac failure
  • loss of vision in one eye from non-arteritic anterior ischaemic optic neuropathy (NAION)
  • severe hepatic impairment
  • hypotension, meaning blood pressure below 90/50 mmHg
  • recent stroke or myocardial infarction
  • known hereditary degenerative retinal disorders such as retinitis pigmentosa

Caution also applies with anatomical deformation of the penis and with conditions that predispose to priapism, such as sickle cell anaemia. Headache is very common, affecting more than 1 in 10 men. Dizziness, flushing, nasal congestion, nausea, indigestion and colour vision changes are common (Viagra SmPC, 2025). An erection lasting longer than 4 hours needs immediate medical help.

If you get chest pain during sex, have any cardiac history, or your erection problems keep coming back, speak to your GP or call 111 before you take anything. Chest pain that does not ease is a 999 call.

Why it matters: Being anxious about sex does not exempt you from the cardiac and drug interaction checks a prescriber has to run.

6. Treating the anxiety alongside the tablet works better than either alone

UK and European guidelines recommend combining medication with psychosexual or cognitive behavioural therapy for men with a clear psychological contribution to their ED.

The BSSM guideline says psychosexual therapy is indicated where the patient or partner identifies a significant psychological contribution, and that a combined pharmaco-psychotherapeutic approach may be more effective than either alone (Hackett et al., 2018). The European Association of Urology guideline calls the integration of CBT with pharmacological treatment a best-practice approach with demonstrated empirical efficacy (EAU, 2025). The Pyke review names cognitive behaviour therapy and mindfulness training as the psychological options with the most potential (2020).

The order is flexible. You can start with the tablet and add therapy, or the other way round. The BSSM guideline states that concurrent use of PDE5 inhibitors is not precluded in men engaged in cognitive behavioural therapy (Hackett et al., 2018).

Why it matters: You address the cause and the symptom at the same time, which is what both UK and European guidelines recommend.

7. Where you get it matters as much as the tablet itself

Prescription-strength sildenafil legally requires a UK prescription, and the MHRA seized around 19.5 million doses of illegal erectile dysfunction medicines between 2021 and 2025.

The figure is from an MHRA press release of 13 February 2026, which warned that unauthorised products may contain no active ingredient, the wrong dose, hidden drugs or toxic ingredients. Its advice: use only UK-registered pharmacies and check them on the General Pharmaceutical Council register.

The check takes a minute. Find the registration number on the site, search it at pharmacyregulation.org/registers/pharmacy, and leave if it is not there. The GPhC calls “no prescription required for prescription medicines” a tell-tale sign of a fake pharmacy, and notes that advertising prescription-only medicines and prices on a home page is illegal under regulation 284 of the Human Medicines Regulations 2012 (GPhC guidance, retrieved September 2026).

One legal route needs no prescription. In November 2017 the MHRA reclassified Viagra Connect, a 50 mg sildenafil tablet, to a pharmacy medicine sold after a discussion with the pharmacist (MHRA, 2017).
